Das Arthrogrypose-Nierenfunktionsstörung-Cholestase (ARC)-Syndrom
neurogener Arthrogryposis multiplex congenita, gestörter Nierentubulusfunktion und
neonataler Cholestase mit niedriger Aktivität der Gammaglutamyl-Transferase im
Serum.
Der Phänotyp ist unterschiedlich,
isolierter renaler tubulärer Azidose bis zum voll ausgeprägten Fanconi-Syndrom
(Polyurie, Aminoazidurie, Glykosurie, Phosphaturie und Bikarbonatverlust).
Leber:, Hypoplasie der intrahepatischen Gallengänge und Ablagerung von Lipofuszin.
Schwere Gedeihstörung, Thrombozyten-Dysfunktion (mit evtl. schweren Blutungen),
faziale Dysmorphien (tiefsitzende Ohren, schlaffe Haut, hoher Gaumenbogen,
schnabelförmige Nase, kleine vordere Fontanelle), Diarrhoe, rezidivierende
Fieberschübe, Hirnfehlbildungen und Schallempfindungs-Schwerhörigkeit. In 75% der
Familien wurden ursächliche Mutationen im VPS33B-Gen (15q26.1) gefunden.
Die meisten Patienten versterben innerhalb des ersten Lebensjahres, trotz Behandlung
der metabolischen Azidose und der Choletase. Länger überlebende Patienten haben
eine Leberzirrhose und sind schwer entwicklungsretardiert.
