Anurie• Definition: Urinausscheidung

< 100 ml/24 h

• postrenale• (“urologische”) Ursachen:

-Obstruktion der ableitenden Harnwege

-supravesikal-infravesikal

• Diagnostik: Sonographie

• Therapie:– Entlastung der Nieren (UK,

JJ, PCN)– Ableitung der Harnblase

(DK, Zystofix)

• Prospective, randomisierte, doppel blinde, multicenterStudie

• Ketorolac beladener Stent (LexingtonTM - Boston Scientific) vs PercuflexR Plus stent

• Ketorolac trimethamin 13% im Polymer• 276 Patienten 14 Standorte, Stent nach URS für 4-10

Tage• Studienendpunkt: • primär Erhöhung der Schmerzmitteldosis, Klinikbesuch,

DJ entfernung• Sekundär: Intervention für Schmerz, VAS Schmerz

(Mikrohämaturie, Krea , Leukozytose, Harnsäure)

• Sono, Rö-Abd., IVP(bei Schmerzfreiheit: caveFornixruptur)

• Nativ-CT (Nachweis röntgennegativer Steine)

Pyelonephritis• Fieber, Zylinder, BKS• DMSA scan• Nierenparenchyminfektion +/- Reflux

Risiko• 17% der Kinder mit asympt. Bakteriurie

haben Narben• Sympt. HWI: 4,5% Narben

Hypertonus

• häufigste Ursache der Obstruktion am Blasenauslass (1:500-800)

• antenatale bilateral Hydronephrose• vergröß erte, wandverdickte Blase• MCUG: dilatierte Prostata, Reflux• trabekulierte Blase• „hoher“ Blasenhals

Obstructive Uropathy

• bilateral hydronephrosis ,• enlarged bladder with dilated

posterior urethra• thickening of the bladder wall • oligo - oder anhydramnion

fetal intervention -fetendo

• amniotic fluid from fetal urine• from 16th gestational week• lung development from 16-28th week• no devlopment of bronchial tree ,

pulmonary hypoplasia• posterior urethral valves 1.8000 male

infants (the earlier diagnosed, the more severe)

• fetendo : mortality 43% (M.Harrison, UCSF)

• 36 y. primipara, 26th. week• fetus: bilateral hydronephrosis, distended

bladder, dilated posterior urethra• caryotyping from amniotic fluid : 46XY• 2x fetal bladder puncture: hydronephrosis

resolved• fetal urinary bladder puncture: good prognosis

electrolytes (Sod. 102 Eq/l, Chl. 82 Eq/l, osm 215 mOsm/l, ß2 glo bulin< 40ug/l)

• renal ultrasound : normal echogenity, no cortical cysts

2.6mm endoscope over wire, inspection of bladder and posterior urethra

• 0.035 inch wire adanced through penis• 2.8 F JJ stent placed between amnion cavity

and bladder under ultrasound control
