Die mit Frau Prof. M. Scherr besetzte Professur für molekulare Hämatologie befasst sich in enger Kooperation mit Prof. Eder mit der Regulation der Genexpression durch kleine nicht-kodierende RNAs (RNA- Interferenz, miRNAs) und untersucht mittels lentiviralem Gentransfer Differenzierungsvorgänge in normalen und malignen hämatopoetischen Stammzellen (Arbeitsgruppenleiter: Prof. Scherr, Prof. Eder). Diese Arbeitsgruppe hat auf ihren Gebieten methodologisch einen Vorsprung, der weltweite Kooperationen begünstigt. Weitere in der Leukämieforschung aktive Arbeitsgruppen befassen sich mit der Bedeutung von MN1, WT1, MLL5 und weiteren potentiellen Onkogenen (Dr. M. Heuser, Dr. F. Damm) sowie mit gestörten Signalwegen bei der malignen Entartung (PD Dr. Reuter, Dr. rer. nat. Venturini) bzw. mit der Bedeutung von gestörten Protein/DNA-Interaktionen in der Differenzierung (PD Dr. Krauter, Dr. K. Wagner). Diese präklinischen Untersuchungen werden komplementiert durch die großen klinischen multizentrischen Studien zur akuten myeloischen Leukämie und myelodysplas- tischen Syndromen, die von der Klinik geleitet werden (Prof. Ganser, PD Dr. Krauter, Dr. Dr. Stadler) und die von Untersuchungen zur Modulation des knochenmarkständigen Immunsystems begleitet werden (Prof. Mischak-Weissinger, Dr. K. Püllmann). In der Transplantationsforschung ergänzen sich präklinische Projekte zur GvHD und GvL (Prof. Mischak-Weissinger, Prof. Franzke) und klinische Studien zu neuen Konditionierungsverfahren (Prof. Mischak-Weissinger, Prof. Ganser, Dr. Buchholz, PD Dr. Krauter, Prof. Eder). Die prädiktiven Proteomanalysen bei stammzelltransplantierten Patienten (Prof. univ. Mischak-Weissinger) sind international wegweisend. Innerhalb des IFB-Tx bearbeiten weitere Arbeitsgruppen die Pathophysiologie der GvHD (Prof. Eder, Dr. C. Könecke), des Endothels und des Hämostasesystems (Dr. A. Trummer). Im Rahmen der Exzellenzinitiative REBIRTH untersucht die Arbeitsgruppe von Frau Prof.’in Dr. Stripecke den Einsatz lentiviral programmierter dendritischer Zellen zur tumor- und infektionsspezifischen Therapie in präklinischen Modellen. Schwerpunkte der hämostaseologischen Forschung sind innerhalb dieses größten Zentrums in Norddeutschland Untersuchungen zur Entstehung und Behandlung der Hemmkörperhämophilie (Dr. Tiede, Dr. Eisert), der Thrombozytenfunktionen (Dr. M. Varvenne) und die klinische Versorgung von Patienten mit seltenen angeborenen und erworbenen Störungen der Blutgerinnung. Die onkologische Forschung konzentriert sich auf translationale Forschung (Dr. Grünwald, PD Dr. Reuter) sowie klinische Phase I-III-Studien z. Forschungsprofil

Die Abteilung forscht anwendungs- und grundlagenbezogen in den Bereichen der Hämatologie (hier insbesondere Leukämie- und Transplantationsforschung), Hämostaseologie und Onkologie. Die mit Frau Prof. M. Forschungsprojekte

Risk prediction and target identification in acute myeloid leukemia (AML)

Despite advances in our understanding of the cellular and molecular basis of leukemia, the standard treatment of AML continues to rely on a 30 year old, side-effect rich chemotherapy regime with cytarabine and anthracycline, producing long-term survival rates of 20-30%. Major improvements in treatment can be envisaged with the development of new strategies that specifically impair the functional properties of leukemic stem cells by stimulating their differentiation and attendant loss of self-renewal and/or that target the self-renewal machinery directly. Our approach to improve the understanding and treatment of AML is to develop and characterize new disease models, and to test novel treatment approaches in these models. Gene expression profiling has been extensively used during the last years to characterize the transcriptome of AML cells. To identify genes relevant for primary drug resistance we compared gene expression profiles of leukemic cells from patients with good or poor response after the first induction chemotherapy. We identified a set of genes that was upregulated in drug resistant patients and was significantly associated with genes overexpressed in normal hematopoietic stem cells (Heuser et al. Haematologica 2005). One of the genes most highly upregulated in this signature was meningioma 1 (MN1), a highly conserved gene without apparent homologies to other known proteins. The meningioma (disrupted in balanced translocation) 1 (MN1) gene was first identified as a fusion partner of TEL, an ETS transcription factor, in patients with myeloid leukemia or myelodysplastic syndrome containing the translocation t(12;22)(p13;q11). MN1 is expressed at high levels in hematopoietic progenitor cells, and is rapidly downregulated during differentiation to mature myeloid or erythroid cells. We found that high MN1 expression is associa-ted with a poor prognosis in a cohort of 142 AML patients with a normal karyotype (Heuser et al. Blood 2006). We have demonstrated using retroviral gene transfer and bone marrow transplantation that MN1 overexpression was sufficient to induce a rapidly lethal AML in mice (Figure 1A and 1B). Engineered overexpression of MN1 in bone marrow cells also readily gave rise to immortalized cy-tokine dependent cell lines of an immature myeloid phenotype that were transplantable and highly leukemogenic. We found strong evidence that MN1 blocks differentiation by repressing transcription of RAR� target genes in hematopoietic cells. Specifically, overexpression of MN1 in a murine cell line model increased resistance to all-trans retinoic acid- (ATRA) induced differentiation by >3,000-fold (Figure 1C). Interestingly, in a clinical trial of AML patients older than 60 years treated with standard chemotherapy with or without ATRA, patients with high MN1 expression levels did not benefit from ATRA treatment whereas patients with low MN1 expression levels survived significantly longer when treated with ATRA (Figure 1D, Heuser et al. Blood 2007). Thus, MN1 is a key factor that blocks dif-ferentiation in AML cells and it will be interesting whether blocking MN1 can overcome the differen-tiation block in leukemic cells. In an attempt to identify genes and transcriptional networks that are

aberrantly regulated in leukemia several groups have compared DNA methylation, histone methylation or acetylation of leukemic samples with normal cells. A global increase in DNA methylation was found in patients with myelodysplastic syndrome (MDS) and AML, and hypermethylation of specifi c genes has been used as a marker of minimal residual disease. DNA methylation inhibitors have shown durable responses in up to 20 percent of AML and MDS patients which do not qualify for intensive

chemotherapy, leaving 80 percent primarily resistant. Interestingly, MDS patients with a karyotype of monosomy 7 or a partial deletion of chromosome 7 had an increased response rate to decitabine. Recently, we have characterized a loss of function model of MLL5, a member of the MLL gene family that is located on chromosome 7. We found that normal hematopoietic stem cells (HSC) of MLL5 null

mice were dramatically depleted upon decitabine treatment through induction of myeloid differenti-ation, whereas HSCs of wildtype mice were not affected (Figure 1E, Heuser et al. Blood 2008). This observation together with the decitabine response in patients with deletion of chromosome 7 suggest that MLL5 may play an important role in de novo resistance to DNA methyltransferase inhibitors and that MLL5 expression may constitute a novel biomarker to predict the response to DNA methylation inhibitors in patients with AML or MDS. Work in our laboratory will continue to identify and validate biomarkers in AML and to develop strategies for targeted therapies.
