KIEFERORTHOPÄDIEEDITORIAL

Das Ressort Fortbildung der zmk bern startet mit neuen, interessanten und aktuellen Kursthemen ins Fortbildungsjahr 2019. Die Master Kursreihe mit den international viel beachte-ten, 3-tägigen Kursen aus den Gebieten der Implantatchirur-gie, der Implantatästhetik und der Parodontologie, wurde durch einen äusserst gut besuchten Master Kurs über die Implantattherapie im zahnlosen Patienten erweitert. Dieser wird durchgeführt von der Klinik für Rekonstruktive Zahn-

medizin und Gerodontologie mit Prof. Urs Brägger und Prof. Martin Schimmel in Zusammenarbeit mit der Klinik für Oralchirurgie. Die Klinik für Zahnerhaltung, Präventiv- und Kinderzahnmedizin mit Prof. Hendrik Meyer-Lückel bietet erstmals Kurse mit den neuen Möglichkeiten in der Kariesdiagnostik und für den Familien-zahnarzt an.

Damit können wir Ihnen auch dieses Jahr ein thematisch breit gefächertes Ange-bot bieten und garantieren, dass wir unserem Prinzip qualitativ hochstehender und preislich moderater Kurse treu bleiben. Das Angebot umfasst weiterhin rund 20 Kurse und bietet diese als Ganz-, Halbtages- oder Abendkurse an.

Das Buser Implant Symposium im Mai wird einen Rückblick auf 20 Jahre Fortschritte in der Implantologie geben. Mehrere international reputierte Referenten auf diesem Gebiet haben ihr Kommen zugesagt und zeigen damit auf, dass die zmk bern einen ausgezeichneten Ruf geniesst. Dieses Symposium wird mit meiner Abschiedsvorlesung als Klinikdirektor kombiniert, welche am gleichen Abend ebenfalls im Kursaal stattfindet.

Bereits zum 3. Mal organisieren die Klinik für Oralchirurgie und Stomatologie und die Klinik für Parodontologie, mit Prof. A. Sculean, gemeinsam das International Symposium on Regeneration and Esthetics in Periodontolgy and Implant Dentistry.Das hochkarätig besetzte Symposium in den Bereichen der chirurgischen Implanto-logie und Parodontologie wird am Vortag erstmals mit einer Nachmittagssession mit vier Live Operationen ergänzt. Dieser Anlass wird die diesjährige Kongressreihe abrunden und wir erwarten wiederum Teilnehmer aus vielen Regionen dieser Welt.

KurszielEs muss nicht immer alle Karies entfernt werden! Wurde man vor 20 Jahren für diese Haltung noch stark kritisiert, hat sich das «Heal and seal Konzept» zur Behand-lung der Karies wissenschaftlich durchgesetzt und ist inzwischen das Leitmotiv für viele Kolleginnen und Kollegen in der zahnärztlichen Praxis. Zur Umsetzung sind ein grundlegendes Verständnis über die Entstehung und Progredienz der Karies, neue diagnostische Möglichkeiten sowie Therapien notwendig.

Dieser Kurs soll Wissen auffrischen und vertiefen. Im praktischen Teil wird vermit-telt, wie «individualisierte Röntgenhalter» hergestellt werden und die approximale Kariesinfiltration kann am Modell Schritt für Schritt geübt werden.

KurszielSoll ein Retainer neu geklebt oder entfernt werden? Welche Materialien sind zu empfehlen um Nebenwirkungen zu verhindern? Gibt es Alternativen? Diese Fragen stellen sich dem Allgemeinzahnarzt bei der Langzeitretention von Patienten nach kieferorthopädischer Behandlung.

Ein Überblick über die Stabilität nach Behandlung verschiedener Malokklusionen und über diverse Retentionstypen leitet das Thema ein. Anschliessend werden an Beispielen Grundsätze der Retention erläutert, Nebenwirkungen von Retentions-apparaturen erklärt sowie Problemlösungen dargestellt. Weiter wird gezeigt, wie eine optimale Langzeitbetreuung durch den Allgemeinzahnarzt möglich ist.

Umfeld, EvidenzBei guten Kammverhältnissen und einer mentalen und motorischen Adaptationsfähigkeit ist beim zahnlo-

sen Patienten normalerweise eine Versorgung mittels „normaler“ Prothese(n) als einfache, wirksame und

zweckmässige Behandlung anzustreben.Behandlungsindikationen Zahnloser Kiefer Bei zeitlich gestaffelter Räumung ist eine Aufbauprothese (siehe Teilprothetik 4611), bei Räumung in

einer Sitzung eine Immediatprothese 4601 als Lösung für 2-5 Jahre indiziert.

PlanungsvariantenIm Rahmen der sozialen Zahnmedizin stehen folgende Planungsvarianten zur Verfügung:

o Keine Behandlung, keine Protheseo Kunststoff-Teilprothese mit Drahtklammern / Tarifpos. 4611 / Aufbauprothese

gemäss umstehenden Planungs- und Behandlungsvorgaben.

o Jahr der letzten prothetischen VersorgungAbrechnungZahnarzt: zum zahnärztlichen Sozialversicherungstarif („SV-Tarif“, früher „SUVA-Tarif“)

Zahntechnische Leistungen: sind Sonderanfertigungen gemäss Medizinproduktegesetzgebung, für

welche letztlich der behandelnde Zahnarzt die Verantwortung trägt. Zahntechnikkosten sind deshalb durch

den Behandler in Auftrag zu geben / zu bezahlen / vorzufinanzieren und zuschlagsfrei seinen eigenen

Behandlungskosten anzufügen.Ab 1.1.2010 gilt der Zahntechniktarif gemäss Konkordanzliste EL+SOZ (grüne Spalte) und mit einem

FESTSITZENDE + IMPLANTATGETRAGENE PROTHETIK

Aus Gründen der besseren Lesbarkeit wird im folgenden Text die berufsbestimmende männliche oder weibliche Sprachform ver-

wendet; die andere männliche oder weibliche Form ist jedes Mal automatisch mit eingeschlossen.

Umfeld, EvidenzFestsitzender Zahnersatz und implantatgetragene Kronen sind sehr komfortabel, sehr teuer und entspre-

chen meist nicht den Kriterien einfach - wirtschaftlich - zweckmässig. Festsitzende prothetische Behand-

lungsmittel sind grundsätzlich nur im Ausnahmefall, nur bei sehr guter Mundhygiene und Patientenmitar-

beit und nur bei einer Langzeitprognose von normalerweise mehr als 10 Jahren bewilligungsfähig.

Im gepflegten und kariesarmen Gebiss gibt es zunehmend Behandlungssituationen, welche sich ohne

festsitzende prothetische Mittel nicht vernünftig und nur mit übermässigem zusätzlichem Schaden für das

Kausystem versorgen lassen. Es macht beispielsweise wenig Sinn, eine hochwertige Teilprothese auf

einem nur minimal vorbereiteten Restgebiss zu verankern. Langfristig lassen sich mit solchen Massnah-

men deutlich Kosten sparen.

Gemäss Eidgenössischem Versicherungsgericht, Urteil P 59/05 (29.03.2006), ist eine VMK-Krone zulas-

ten der Ergänzungsleistung nicht bewilligungspflichtig. Eine Krone ist zwar wirksam, ev. sogar zweckmäs-

sig, aber nicht als wirtschaftliche Lösung zu beurteilen. Daher sind die Ausnahmen sehr eng zu definieren

Das Angebot an prothetischen Mitteln ist heute sehr divers und viele der angebotenen Mittel sind nicht

langfristig erprobt. Moderne und wenig erprobte Behandlungsmittel ergeben behandlerspezifisch sehr

unterschiedliche Resultate, d.h. in der Hand eines einzelnen erfahrenen Anwenders ergeben sich langfris-

tig sehr gute Resultate bzw. ist beim unerfahrenen Anwender das Resultat suboptimal. Im Normalfall sind

deshalb langfristig erprobte Behandlungsmittel mit anerkannter Indikation und mit Evidenz für gleichblei-

bend gute Resultate zu bewilligen.

Behandlungsindikationen

Aufbau eines stark zerstörten Einzelzahnes, welcher nicht mittels Füllung restaurierbar ist.

Aufbau von stark zerstörten Ankerzähnen für langfristigen abnehmbaren Zahnersatz.

Versorgung einer Einzelzahnlücke im gepflegten und kariesarmen Gebiß, welche sich nur über-

mässig invasiv und / oder funktionell unbefriedigend mittels abnehmbarem Zahnersatz versorgen läßt.

Dabei muss folgende funktionelle oder ästhetische Indikation gesichert sein:

funktionelle Indikation: Kauunfähigkeit nach Zahnverlust ohne funktionelle Adaptation und mit

weniger als 10 funktionierenden Antagonistenpaaren.

ästhetische Indikation: Verlust von Frontzähnen inkl. Zähne 14 24 aufgrund der aktuellen Planung

oder während der letzten achtzehn Monate.

Planungsvoraussetzung festsitzende Prothetik

Der Patient zeigt ein gepflegtes und weitgehend kariesarmes Gebiss, ist an der oralen Gesundheit inte-

ressiert und arbeitet aktiv seit Jahren mit, diese orale Gesundheit zu erhalten. Dies ist durch den Behand-

ler speziell zu attestieren.

Complianceattest: Attest einer aktiven (positiven) Patientenmitarbeit bezüglich Mundhygiene und von

minimal drei besuchten Mundhygiene-Recallterminen in den letzten 18 Monaten in der gleichen behan-

delnden / planenden Praxis / Klinik.

KurszielAuch heute noch werden viele Totalprothesen in den Privatpraxen angefertigt. Oft sind die zahnlosen Patienten sehr alt und haben einen niedrigen sozio-ökonomischen Status sowie zahlreiche Allgemeinerkrankungen. Folglich ist die Adaptationsfähigkeit oft reduziert – Allgemein- und Tumorerkrankungen, Polypharmazie und Fragilität sind Herausforderungen an den Zahnarzt und den Zahntechniker. Nicht jeder zahnlose Patient möchte Implantate erhalten. Manchmal sprechen auch medizinische oder finanzielle Gründe dagegen. Die klinischen Gegebenheiten stellen häufig eine grosse Herausforderung dar. Daher ist es wichtig, dass die Fertigkeit, Totalprothesen herzustellen, nicht verloren geht und weiter vereinfacht und perfektioniert wird, um die Adaptation an den Zahnersatz zu erleichtern und die Behandlungszeit für die Patienten zu minimieren. Die klassische Totalprothetik ist weiterhin von enormer Bedeutung, um die Grundzüge der zahnärztlichen Ästhetik und der oralen Funktionen zu verstehen.Ziel des Kurses ist es, zahnmedizinisches sowie zahntechnisches Wissen von konventio-nellen Totalprothesen aufzufrischen. Verschiedene mögliche Wege zur funktionellen und ästhetischen Totalprothese werden aufgezeigt, um vorhersagbare Ergebnisse zu erreichen.

Course objectivesThis course is dedicated to the management of edentulous patients.

Edentulism is still a harsh reality for many of our patients. Losing all teeth is the «terminal outcome of a multifactorial process involving biological factors and patient-related factors» (Felton 2009). Edentulism constitutes a major oral disabi-lity in regard to function, aesthetics and psycho-social aspects.

Complete denture prosthetics has come a long way since Gysi, Ackermann or Gerber. Nowadays, there are rational procedures that also allow the less experienced practitioner to provide patients with functional and aesthetic complete dentures in a reduced number of clinical appointments, such as Biofunctional Prosthetic System (BPS). The knowledge of complete denture prosthodontics constitutes the basis for any implant-supported complete prosthesis.

The use of osseointegrated implants for the stabilization of complete dentures is one of the great achievements of modern dentistry. Today, the proposed standard of care for edentulous patients is a conventional mucosa-born denture in the maxilla, and an implant-supported overdenture anchored on two implants in the mandible to ensure patient satisfaction and oral function.

However, many of our patients demand fixed prostheses for optimal comfort and function, but we need to discuss what is feasible in the specific anatomical condition.

The main topic of the course will focus on rational screw-retained fixed implant prostheses on ultra-short 4 mm implants applying the BPS Workflow, virtual planning and guided surgery. The course will also discuss the management of edentulous patients who are often old and may become frail. Finally, the course is complemented with an overview on removable implant prosthetics and Narrow Diameter Implants and Mini Dental Implants.

The course will include two live surgeries, one live prosthetic demonstration, hands-on courses with virtual planning and an insertion of extra-short implants in a jaw model.

Course objectivesThe aim of this course is to provide general dentists and specialists with an evidence based concept on the treatment of periodontally compromised patient. Starting with an overview on the anatomy, microbiology, etiology and pathogenesis, the course encompasses the most important aspects in diagnosis, treatment planning, nonsurgical and surgical therapy, use of systemic and local antibiotics, lasers and photodynamic therapy in the overall frame of a comprehenisve treatment concept. Course objectivesThis course addresses general dentists and specialists who intend to learn more about the use of regenerative and plastic-esthetic procedures in Periodontology. Furthermore, the course will also provide a concept for the treatment of peri-implant infections and treatment of soft tissue defects at implants. A concept based on comprehensive biological research of the last decades is presented. Case presenta- tions and discussions as well as practical exercises and live surgeries help to acquire in-depth knowledge about the use of various surgical techniques aiming to regene- rate the lost periodontal structures and/or to achieve predictable coverage of single and multiple gingival recessions. The biological background for the application of biologic active factors, different types of grafting materials, guided tissue regene- ration or various combinations thereof will be presented and discussed.

Innovative surgical techniques such as the Modified Coronally Advanced Tunnel (MCAT ) and the Laterally Moved Double Tunnel (LMDT) with adjunctive connective tissue grafts, biologically active factors and soft tissue replacement grafts aiming at achieving predictable recession coverage and increase in soft tissue thickness will be presented.

die synoptische Behandlungsplanung und Therapie im Sinne des etablierten Berner

Diagnostik und Therapie von peri-implantären Infektionen. Die TeilnehmerInnen

werden Gelegenheit haben, eigene Fälle zu besprechen und zusammen mit den

DozentInnen geeignete Behandlungspläne auszuarbeiten.

Wichtiger Bestandteil sind die praktischen Übungen zur richtigen Handhabung und

Anwendung der maschinellen und Hand-Instrumente und der Pulverstrahlgeräte,

zu den verschiedenen Lappen- und Nahttechniken für die resektive, regenerative

und plastisch-ästhetische Parodontalchirurgie und die peri-implantäre Chirurgie.

KurszielDas Ziel dieses Kurses ist es, das Berner Konzept der synoptischen Behandlungs- planung detailliert darzustellen und dem praktizierenden Zahnarzt eine evidenz- basierte Übersicht und einen Leitfaden für die vorhersagbare und erfolgreiche Parodontalbehandlung zu geben.

Es werden neuste Forschungsergebnisse aus den Bereichen der parodontalen Diagnostik, Risikobeurteilung, Verhaltenstherapie/Raucherentwöhnung sowie der regenerativen und plastisch-ästhetischen Parodontalchirurgie dargestellt. Innovative und praxisrelevante chirurgische Techniken und neue Materialien zur Regeneration von verschiedenen parodontalen Defekten sowie zur Deckung von singulären und multiplen Rezessionen werden ebenfalls präsentiert. Die verschie-denen Phasen der synoptischen Parodontalbehandlung werden in klinischen Fall-präsentationen erläutert und diskutiert. Der Einsatz von systemischen und lokalen Antibiotika/Antiseptika sowie von verschiedenen innovativen Technologien wie Laser, Pulverstrahl etc. wird kritisch analysiert und Schlussfolgerungen für die Praxis gezogen.

Ein wichtiger Bestandteil des Kurses ist auch die Darstellung und die kritische Analyse der verschiedenen Behandlungsmöglichkeiten von Periimplantitis. Zudem werden diverse Methoden zur Ermittlung der individuellen Risiko-Diagnostik und des so- genannten «Recall-Intervalls» gezeigt, mit welchen die Stabilität der erreichten Ergebnisse über lange Zeit gesichert werden sollen.

Course InformationThis one-week course was established 18 years ago and has become the most successful ITI Education Week offered by the ITI. It was designed for clinicians with an interest in the field of implant dentistry. This basic course presents evidence- based treatment concepts for the rehabilitation of fully and partially edentulous patients with implant supported prostheses. Besides more than 25 lectures, the participants can watch several live surgeries observing how dental implants are applied. The course also offers the opportunity to participate in treatment planning as well as surgical and prosthetic hands-on workshops.

Course objectivesFor more than 20 years, implant esthetics has been a dominating topic in implant dentistry in partially edentulous patients. In the anterior maxilla, the rehabilitation of patients with implant supported restorations is challenging for involved clinicians due to the high esthetic demand expressed by patients, and a difficult anatomical situation with bone and/or soft tissue deficiencies, which are often present in the anterior maxilla. This interactive 3-day CE course has been established more than 18 years ago for experienced colleagues in implant dentistry. It systematically presents the well documented treatment concepts for the handling of single tooth gaps or extended edentulous spaces in the anterior maxilla of partially edentulous patients. These concepts have been developed in more than 25 years by our group providing successful outcomes with high predictability and a low risk for complications.

Course objectivesIn the past 25 years, implant therapy has rapidly expanded in private practice documented by a large number of implant patients every year. This positive deve-lopment is due to various factors, among them a significant progress with bone augmentation procedures. In our department, roughly 60% of implants are placed in conjunction with a bone augmentation procedure today, using either a simulta-neous or a staged approach. The two main surgical techniques for bone augmenta-tion are the GBR technique using a 2-layer composite graft and barrier membranes, and the sinus floor elevation (SFE) procedure using either the lateral window tech-nique or the transalveolar Osteotome technique. Both augmentation procedures are routinely used in our department for more than 20 years to overcome local bone deficiences. Both techniques provide excellent regenerative outcomes and long-term stability with high predictability as documented by numerous clinical 10-year studies.

This interactive 3 day CE course has been established 19 years ago for experienced implant surgeons. The speakers will present the biological basis for current surgical procedures in various lectures, and the criteria for the selection of appropriate biomaterials. State-of-the-art information will also be provided on 3D preoperative analysis of implant patients with anatomic borderline situations using CBCT. The different surgical techniques will not only be presented and discussed in various lectures, but also demonstrated with 5 to 6 live surgeries. In addition, two hands-on workshops are offered.

Course objectivesIn the past 10 years, esthetic implant complications and failures have significantly increased. Most of these failures are caused by inproper treatment quality of involved clinicians, in particular the implant surgeon. The most important cause for esthetic failures is the implant malposition or malaxis, followed by much less frequent causes, such the wrong number of implants, aggressive surgical protocols which overstress the healing potential of the tissues, or cemented implant restora-tions, which cause peri-implant infections due to excess of cement.

This 3-day CE course will first present the treatment concepts how to deal with esthetic disaster cases, how to remove osseointegrated implants, how to correct missing keratinized mucosa, and how to retreat these patients with implant supported prostheses. On the third day, the Bernese Implant Protocols, from a surgical and prosthetic aspects, will be demonstrated how to prevent esthetic implant complications. Besides more than 15 lectures, 5 to 6 live surgeries will be presented to document important steps of therapy in various clinical situations.
