Retinoide- 20-30% passagere, milde Elevation der LFP1
~1% akute Hepatitis2
Amoxicillin/Clavulansäure3
- Inzidenz: 3-17/1000004, (14% DILI registry)3
- 6%-36% hepatozelluläres Muster (Alter, Therapiedauer)3
- Therapiebeginn bis Ikterus (mean 16 Tage)- 50% lange Latenz (15 [2-55] Tage nach Absetzen)- Normalisierung der LFP nach 11.5 Wochen5
- 2.9 % Tod/OLT (akutes/subakutes Leberversagen)3,5 - HLA DRB*1501 – Assoziation6
Zusammenfassung
Vor Therapie mit potentiell hepatotoxischem Medikament- Präexistente Lebererkrankung? (HBsAg Carrier)- Genetische Prädisposition (Familienanamnese)- Komedikation – Interaktionen - Alkohol
Erhöhung der LFP unter Therapie- Ausschluss von interkurrenten Lebererkrankungen- Latenzzeit bis Auftreten – Signature (ALF) - Autoimmunität ausschließen (Minozyklin)
Therapeutische Optionen- Dechallenge, No Rechallenge in severe Cases !!- N-Acetylcystein/Glukokortikoide?/UDCA?- OLT/MARS?
