Chemotherapie bei frühem triple-negativem Brustkrebs



Rationale I

Bislang existieren keine biologisch zielgerichteten Therapien zur Behandlung des primären triple-negativen Mammakarzinoms (TNBC)

Die konventionelle Chemotherapie ist immer noch die einzige Standardtherapie

TNBC → hohe Anzahl von TILs, die mit einem verbesserten Ergebnis korrelieren1,2,3

Die I-SPY2-Plattform: signifikante Erhöhung der pCR-Raten (von 20% auf 60%) durch die Zugabe von Pembrolizumab zur Standard-Chemotherapie hin1

Phase Ib KEYNOTE-173-Studie: pCR-Raten von bis zu 60%, wenn Pembrolizumab der NACT bestehend aus Anthrazyklinen, Taxanen und Carboplatin hinzugefügt wurde2

GeparNUEVO-Studie: nummerisch, aber nicht statistisch signifikant höhere pCR-Rate (von 44% im Vergleich zu 53%), in dem Durvalumab zu einer Anthrazyklin/Taxan-basierten Chemotherapie hinzugefügt wurde3

Primäre Zielkriterien

1. Pathologische Komplettremission in der Brust und Lymphknoten (ypT0/ypN0)

Vergleich der histologisch ermittelten pathologischen Komplettremissionsrate in der Brust und in den Lymphknoten zwischen triple-negativen Brustkrebspatienten, die mit oder ohne Durvalumab in Kombination mit Chemotherapie behandelt werden.

2. Ereignisfreies Überleben (EFS)

Es soll bestimmt werden, ob die Hinzugabe von Durvalumab zu einer Chemotherapie gefolgt von einer adjuvanten Gabe von Durvalumab (vs. Beobachtung) das ereignisfreie Überleben bei Patienten mit triple-negativem Brustkrebs verbessert.

Einschlusskriterien (Auswahl)

Histologisch durch Stanzbiopsie gesichertes, unilaterales oder bilaterales primäres Mammakarzinom

Erkrankungsstadien: – IIA: T2 N0 M0, T1 N1 M0

– IIB: T2 N1 M0, T3 N0 M0

– IIIA: T0–2 N2 M0, T3 N1–2 M0

– IIIB: T4 N0–2 M0

– IIIC: T1-4 N3 M0

Zentral bestätigtes triple-negatives Mammakarzinom mit ER-negativ (<10%)/PR-negativ (<10%)/HER2-negativ und zentral bestätigten Ki-67 Status; TILs



GeparTreize: Ausschlusskriterien (Auswahl)

(Auto)Immunerkrankungen

Schilddrüsenerkrankungen: TSH normal

klinisch aktive Turkulose, Hepatitis B/C, HIV – Testung für Hepatitis B und C, HIV zum Screening

Maligne Vorerkrankung – Patient muß seit 5 Jahren krankheitsfrei sein oder hat ein geringes Rezidivrisiko

– CIS der Zervix, Basalzellkarzinom der Haut



Globale Studie mit mehreren Ländern

Anzahl Studienzentren: global 250, davon in DE – ca. 50

Patientenzahl: 1528 global, davon in DE - ca. 350

Zeitplan: – Patientenrekrutierung: 24 Monate (Q-II 2019 – Q-II 2021)
