• In the high risk patient with postoperative AF, such as those with a history of stroke or TIA, the routine use of heparin should be considered

44-61% aller Arrhythmien, Häufung Tag 1-3
• Bei kritsch Kranken Erfolgsrate der DC CV u.U. sehr gering (as low as 35%) und nicht anhaltend
• Guidelines favorisieren bei unkompliziertem AF die Frequenzkontrolle
• Frequenzkontrolle nicht schlechter
• Frequenzkontrolle mit ß Blockern, Ca2+ Antagonisten, Amiodarone

Wrap it up (2)
• medikamentöse Rhythmuskontrolle mit Amiodarone, Ibutilide oder Vernakalant
• Vernakalant hohe Konversionsraten und rascher Wirkungseintritt (8-14min) bei kurz bestehendem Vorhofflimmern, signifikant besser als Amio
• Elektrische CV bei hämodynamischerInstabilität, CHF, Präexcitation
• Amio Vorbehandlung erhöht bei kritisch Kranken NICHT die Erfolgsrate der DC CV
