Derentscheidende Vorteil ist die V- förmige Einkerbung in derMitte des Hebels mit den außenliegenden scharfen Spitzenam Arbeitsende. Das Instrument wird parallel zur Längsachsedes zu entfernenden Zahnes oder Wurzelrestes zwischenKnochen und Zahnwurzel zirkulär rotierend eingeführt. Dabeiwird das Arbeitsende mit dem Pinzettengriff der freien Handam Zahn fixiert, so dass die Spitzen des Arbeitsendes an derWurzel entlanggleiten. Wichtig bei der Luxationsbewegungist die ausreichende digitale Abstützung des alveolärenBereichs durch den Behandler. Durch die Zweipunktführungder Spitzen an der Wurzel wird das Abrutschen in denKnochen, im Vergleich zu den herkömmlichen Hebeln mitabgerundetem Arbeitsende erschwert. Dadurch wird der zuextrahierende Zahn oder Wurzelrest wesentlich aktiver undsicherer luxiert, so dass eine operative Entfernung (sog.Aufklappung) vermieden werden kann.Die Form der Luxationsbewegung ist bei jedem Zahn sehrindividuell und ist insbesondere von der anatomischenWurzelform abhängig. Eine Orientierung am präoperativenRöntgenbild kann als Hinweis auf die günstigsteLuxationsrichtung dienen. Dennoch bestehen immer wiederindividuelle Unterschiede, daher ist vor allem das Gefühl desBehandlers gefragt, die jeweils leichteste Luxationsrichtungzu erspüren. Nur so lassen sich Gewebeschäden undDrucknekrosen des alveolären Knochens reduzieren.

Vorteile:

• geringeres Risiko für Nachbarzähne und Zahnersatz, da eine Abstützung nicht notwendig ist

• geringeres Risiko einer perforierenden Weichteilverletzung durch Abgleiten des Instruments

Modified lever according to SchemmelThe modified lever according to Schemmel combines theadvantages of a lever with those of a root tip elevator. Thekey advantage is the V-shaped indentation in the center ofthe lever with the outward-facing sharp tips at the workingend. The instrument is inserted in a circular rotating mannerparallel to the longitudinal axis of the tooth or root rest to beremoved between the bone and the tooth root. In doing so,the working end is fixed to the tooth with the tweezer handleof the free hand so that the tips of the working end slidealong the root. Important for the luxation movement is thesufficient digital support of the alveolar area by thepractitioner. The two-point guidance of the tips at the rootmakes the slipping into the bone, compared to theconventional levers with a rounded working end, moredifficult. Der Kunststoff wird mitPulver und der Anmischflüssigkeit im Anmischbechervermischt, mit einem Heidemannspatel auf die Kroneaufgetragen und die Kronenentfernungszange angesetzt.Nach einer Aushärtezeit von ca. 20 Sekunden kann dieKrone vom Stumpf entfernt werden.Als Alternative kann ein lichthärtendesEinkomponenten-Füllungsmaterial eingesetzt werden;dieses muss mit der Polymerisationslampe ausgehärtetwerden.
