Die Blutentnahme
Das Entnahmesystem Die Wahl des Entnahmeröhrchens richtet sich stets nach dem jewei-ligen Untersuchungsparameter. Es stehen sowohl geschlossene als auch offene Entnahmesysteme mit oder ohne Zusatz von gerinnungs-hemmenden (EDTA, Citrat, Heparin) oder Glykolyse hemmenden (Nat-riumfluorid) Stoffen zur Verfügung:
Blut-, Serum-, Plasma- und Harnproben
SerumSerumproben können aus Vollblut nach vollständiger Gerinnung ge-wonnen werden. Bitte entnehmen Sie dafür Vollblut ohne Zusätze (An-tikoagulanz) und lassen es bei Raumtemperatur mindestens 30 Minuten stehend gerinnen. Nach dem Gerinnungsvorgang und der anschließenden Zentrifugation entsteht das Serum als Überstand. Das Serum direkt im Anschluss durch vorsichtiges Abpipettieren in ein steriles, neutrales Röhrchen überführen.
Folgende Reihenfolge bei der Entnahme von Blutproben empfohlen:
Vollblut für Serum, Citrat-Blut, Heparin-Blut, EDTA-Blut, Fluoridblut und weitere.
Bitte beachten Sie:Alle Vollblutproben ohne Zusätze oder mit Zusätzen (EDTA, Heparin, Citrat) bitte niemals einfrieren! Beim Auftauen kommt es zu vollstän-diger Hämolyse, sodass das Material nicht verwendet werden kann. Nur abzentrifugiertes Serum oder Plasma nach Überführung in ein neutrales Röhrchen einfrieren!

Urin-Keimzahlbestimmung
EDTA-BlutEDTA-Blut wird für die Untersuchung von zellulären und intrazellu-lären Bestandteilen des Blutes, für genetische Untersuchungen und PCR-Untersuchungen hämatogen streuender Infektionserreger ein-gesetzt. Es eignet sich jedoch nicht für Gerinnungsuntersuchungen und die Bestimmung bestimmter klinisch-chemischer Parameter wie Elektrolyte, da EDTA zweiwertige Ionen bindet. Für zelluläre Analysen weist EDTA-Blut eine begrenzte Stabilität von ca. 24 Stunden auf.
EDTA-PlasmaAnders als Serum, welches nach der Blutgerinnung durch Zentrifu-gation gewonnen wird, enthält Plasma durch den Zusatz von Antikoa-gulantien im Röhrchen noch alle Gerinnungsfaktoren. Tiefgefrorenes EDTA-Plasma ist das empfohlene Untersuchungsmaterial für die Bestimmung von Hormonen und Substanzen, die einem besonders schnellen Abbau unterliegen.
Citrat-BlutCitrat-Blut wird für die Parameter der Gerinnungsanalyse (z.B. Quick-Wert, aPTT, Fibrinogen) benötigt. Sollte ein unverzüglicher und ra-scher Transport in das Labor nicht einwandfrei zu gewährleisten sein, empfiehlt es sich Citrat-Blut zu zentrifugieren und das gewonnene Citrat-Plasma in einem sterilen, neutralen Röhrchen tief zu frieren.
Natrium-Flourid-BlutNatrium-Fluorid-(NaFl)-Blut blockiert den Zellstoffwechsel in den Erythrozyten durch Hemmung der Glykolyse, sodass die Konzentration verschiedener Substrate, wie Glucose oder Laktat stabil gehalten wird. Aus diesem Grunde werden Glucose und Laktat stets aus NaFl-Blut-proben ermittelt.
Heparin-PlasmaDer Gerinnungshemmede Effekt von Heparin beruht auf einer Steige-rung der Antithrombinaktivität. Obwohl Heparin-Plasma zur Bestim-mung von klinisch-chemischen Parametern eingesetzt werden kann, (bitte beachten Sie Ausnahmen!) ist Serum für die klinische Chemie dem Heparin-Plasma vorzuziehen.0
HarnFür die Analyse von Harnparametern können Harnproben auf unter-schiedlichen Wegen gewonnen werden (Spontan-, Katheter- oderZystozenteseharn). Die Harnprobe sollte in ein steriles Uringefäß (ohne Stabilisator, bruch- und auslaufsicher) gegeben und das Probengefäß zeitnah ins Labor transportiert werden. Lagern Sie die Harnprobe bis zum Transport gekühlt bei 4-8°C. Harnproben, welche aus der Umwelt aufgenommen wurden sind ungeeignet!
Der ProbentransportProbenmaterialien sollten stets auf dem schnellstmöglichen Wege in das untersuchende Labor transportiert werden. Bitte achten Sie auf eine adäquate Kennzeichnung der Proben und Anforderscheine. In Ab-hängigkeit vom Probenmaterial kann der Transport bei Raumtempera-tur erfolgen oder muss für die gesamte Dauer gekühlt oder tiefgekühlt durchgeführt werden. Tiefgefrorene Proben dürfen während des ge-samten Transportes in das Labor nicht auftauen!
