Mit 43 Kapiteln und einer Autorenkollektion wie aus dem »who is who« präsentiert sich ein aktuelles Standardwerk zum Behandlungsspektrum thermi-scher Traumen in Deutschland.
Nach einer kurzen Einführung zur Historie, Epi-demiologie und Erstversorgung mit Triage, Evalua-tion und notwendigen Transportmedien folgen Empfehlungen zur Struktur- und Prozessqualität für Spezialabteilungen von Kindern und Erwachse-nen: Der Primärversorgung mit ihren besonderen intensivmedizinischen Maßnahmen, chirurgischen Techniken und Lagerungsmodi werden pathophy-siologische und klinische Fakten hinterlegt. Großer Raum wird den Hautersatztechniken und der Dif-ferenzialtherapie mit aktuellen Materialien belas-sen. Für die Läsionen der Grade III und IV sowie das Komplikationsmanagement von konsekutiven nekrotisierenden Infektionen folgen anhand über-zeugender Fallbeispiele die plastisch-rekonstruk-tive Techniken zum Erhalt von Form und Funktion inklusive von Amputationsmöglichkeiten der obe-ren und unteren Extremität. Ein Zukunftsaspekt demonstriert die Potenziale der Allografttrans-plantationen von Hand und Gesicht. Für den klini-schen Alltag werden Ergotherapie, Physiotherapie, Schmerzbehandlung, psychologische Betreuung und sekundäre Korrekturen beim Narbentrauma vorgestellt. Eine ausführliche Hilfestellung zur Dokumentation und Begutachtung Brandverletz-ter soll zur fairen Kompensation beitragen.
Insgesamt finden wir eine profunde multidiszipli-näre Sammlung zu allen Facetten des thermischen Traumas, die Autorenvielzahl lässt dabei unter-schiedliche Stile und Nuancen zu. Soweit Richt-linien oder prospektive Studien andauern, bleiben für den klinischen Alltag zumindest vernünftige Hand lungsempfehlungen. Zur Verminderung der immer noch häufigsten Todesursache bei Brand-verletzten, der Sepsis, sollte das Handbuch in keiner chirurgischen Klinik oder pädiatrischen Notaufnahme fehlen. Es wird fraglos die kritische Reflexion und frühzeitige Verlegung in Spezial-zentren überzeugend anregen.
